Employee Vaccination Incentive Program Begins; Get Vaccine & Flu Shots at Nov. 4 Event

Cincinnati State’s Employee Vaccination Incentive Program started on Tuesday, Oct. 26.

  • President Monica Posey announced the program in an email message to all employees, and thanked employees for helping to fight the spread of COVID-19 in our campus community.
     
  • Employee Incentive prize drawings will start Monday, Nov. 1, and continue through Dec. 20.

How to enter:

Employees who are fully vaccinated, or have received their first dose of a 2-dose COVID-19 vaccine, can enter the weekly prize drawings.

Only employees who are fully vaccinated are eligible for the $5,000 Grand Prize drawing.

  • To enter, fill out the Employee Vaccination Incentive eForm (must be logged in to Blackboard/MyServices) with your name, and when and where you were vaccinated.
     
  • Attach to the eForm scanned copies of both sides of your vaccination card, or bring your vaccination card to Falonda Rodgers or Dina Henderson in the Human Resources Office on Clifton Campus (Main 186) for verification.
     
    (Note: some participants have reported problems attaching their scanned vaccination card to the eForm. The technical problem is being investigated, but if you’re unable to attach your scanned document, please bring your card to Falonda or Dina in HR.)

When your vaccination info has been verified, you’re eligible for weekly prizes that include:

  • Visa gift cards
  • Queen City gift cards for dinner/entertainment
  • Family dinner gift cards
  • Ticketmaster gift cards
  • 1-year fitness club memberships

Prize Drawing Dates:

Each week from Nov. 1 to Dec. 20, eight fully-vaccinated or partially-vaccinated employees will win prizes in the weekly drawing.

On Dec. 20, one fully-vaccinated employee will win a grand prize of $5,000.

  • Weekly winners will be announced on Mondays about 11 a.m.
     
  • Winners can pick up their prizes on Tuesdays between 11 a.m. and noon in the Human Resources Office (Main 186), and then go to the Executive Offices (ATLC 343) to have their photo taken.
     
  • The grand prize winner will receive the $5,000 prize on Dec. 21.

Not yet vaccinated?

Get your first dose of the Pfizer COVID-19 vaccine at the Clifton Campus Vaccination Clinic (facilitated by Walgreen’s) on Thursday, Nov. 4, from 11 a.m. to 3 p.m., in Main 180.

The clinic is free for Cincinnati State employees and students.

This clinic will also provide flu shots and Pfizer COVID-19 second-dose shots and booster shots for those eligible.

  • You can fill out the eForm and enter weekly drawings after you receive your first dose of a two-dose COVID-19 vaccine.
     
  • After you get your second dose of a COVID-19 vaccine, take your completed vaccination card to Falonda Rodgers or Dina Henderson in the Human Resources Office, and then you’ll be eligible for the grand prize drawing.
